Post Discharge Close Follow Up Available for the following:

Patient seen in ER for dehydration and discharged from ER- If they are instructed for close follow up with their PCP, we can see them in between being discharged from ED and seeing their PCP.

Patients being discharged from hospital needing close outpatient follow up- If patient discharged with instructions to follow up with PCP or with referral to nephrologist, we can bridge the time between discharge and being evaluated by their routine healthcare provider.

Patients with renal function changes within range for outpatient setting - if recent blood levels demonstrate renal function changes that are within a range for outpatient management, we can provide treatment and recheck levels and send back to you.

1) All staff are ER providers and nurses- your patient receives ER level care.

2) We can run onsite POC BMP- we can check renal function during their appointment and forward to appropriate provider

3) We perform and draw bloodwork. We can run any requested blood samples and have results forwarded to you or directly to the patient’s specialist for follow up.

4) We are open 7 days a week- so even if your patient is discharged or needs evaluation on the weekends, we have you covered.
