At Purifi IV, we prioritize treatment methods that offer safety, flexibility, and consistent hormone levels. While testosterone pellets are a form of hormone replacement therapy, we have chosen not to offer them for several reasons:
Invasive Procedure: Pellet therapy requires a minor surgical procedure to insert the pellets under the skin. This carries risks such as infection, bruising, and discomfort at the insertion site.
Hormone Level Fluctuations: Pellets release testosterone over time, but this can lead to peaks and valleys in hormone levels. Such fluctuations may result in mood swings, energy inconsistencies, and other side effects.
Lack of Dosage Flexibility: Once inserted, pellets cannot be adjusted or removed easily. If side effects occur or dosage adjustments are needed, options are limited until the pellets naturally dissolve over several months.
Potential Side Effects: Some patients may experience side effects such as acne, hair loss, or increased aggression due to the higher doses delivered by pellets.
Given these considerations, we believe that testosterone injections and topical creams offer a safer and more controllable approach to hormone replacement therapy. These methods allow for precise dosing and easy adjustments, ensuring optimal results and patient comfort.

At Purifi IV, we prioritize not just your current health but your long-term goals — including family planning. If you are actively trying to conceive or plan to have children at any point in the future, we do not recommend starting testosterone replacement therapy (TRT) or hormone replacement therapy through our clinic.
While we do utilize advanced protocols that can help preserve fertility, such as medications that stimulate the body’s natural hormone production, testosterone therapy itself can suppress sperm production and may impact fertility.
That said, we are happy to:
Evaluate your bloodwork thoroughly
Discuss symptoms and hormone optimization goals
Explore non-TRT alternatives that support hormonal health without compromising fertility
However, if having children remains a future goal, we strongly recommend consulting with a fertility and hormone specialist who can guide you through safe, fertility-conscious treatment plans.

No — Purifi IV is not a primary care provider and does not replace your existing relationship with a primary care physician (PCP). While we specialize in hormone optimization and wellness services, including testosterone replacement therapy (TRT), we do not provide general medical care such as:
Managing chronic conditions (e.g., hypertension, diabetes)
Annual physicals
Emergency or urgent care
Routine vaccinations or screenings
We strongly encourage all clients to maintain an active relationship with their PCP to ensure comprehensive care. If you do not currently have a primary care provider, we’re happy to refer you to a trusted local physician.

At Purifi IV, we believe in offering clear and accessible pricing for all our services. Here’s an overview of our TRT pricing structure:
Initial Visit & Bloodwork
Cost: $250
This includes a comprehensive blood panel (more thorough than standard clinics) and an initial consultation with our medical provider.
This is a one-time fee, bloodwork results needed before scheduling your first appointment to determine eligibility.
Monthly Subscription for TRT
For TRT at Home:
Cost: $150/month
Includes bloodwork as needed and prescription management. Bloodwork can be completed at Purifi IV location, or a blood test kit can be sent to your home. Kits sent to your home will need to be drawn at a lab or phlebotomy company (not included in price). When kit is ordered, our third party partner will provide blood draw options based on your location.
Does not include the cost of testosterone.
Month-to-month payment, no refunds once the prescription is sent.

Payment & Subscription Details
The initial evaluation payment must be completed and bloodwork results obtained in order to schedule appointment with healthcare provider. Once bloodwork completed, a link to schedule appointment with healthcare provider will be sent to you.
Subscriptions are renewed monthly, starting from the first of each month, to ensure continuous treatment and access to our services.
All payments for monthly subscriptions must be processed prior to any treatment, ensuring we have the most up-to-date information for your care.
How The Program Works
TRT at Home
Experience the flexibility and convenience of Testosterone Replacement Therapy in the comfort of your own home.
Step 1: Initial Consultation and Bloodwork
Comprehensive Bloodwork: Before starting TRT, you’ll need to undergo a thorough blood test to determine your eligibility. You can have this done either at Purifi IV, a test kit can be sent to your home, or testing done at a lab of your choice. Note: Labs performed at outside lab is not included with pricing. Included labs with pricing are though Purifi IV Suwanee location, or by test kit sent to your home (phlebotomy charge for kits sent to your home are not included and must be paid by you. This is due to testing regulations with our third party partner).
Initial Evaluation: After receiving your bloodwork results, you'll have a consultation with our medical provider to review your results and discuss potential treatment options.
Step 2: Qualification for TRT
Based on your bloodwork and consultation, our medical provider will determine if you qualify for TRT at home.
If you qualify, Testosterone will be sent to you through our pharmacy partner from Purifi IV or a prescription sent to the pharmacy of your choice.
Step 3: Monthly Subscription
Cost: $150/month
Your monthly subscription includes regular follow-up bloodwork, ongoing guidance, and prescription management.
The subscription is month-to-month, and you’ll continue receiving your monthly TRT prescription.
Note: The subscription does not include the cost of testosterone.
Step 4: Regular Monitoring
Stay connected with your provider through phone and/or text for any questions or adjustments to your treatment plan.
You’ll also receive follow-up bloodwork to track your progress and ensure optimal results.
TRT At In-Clinic At Purifi IV
For those who prefer in-person care, we offer weekly TRT injections administered directly at Purifi IV.
Step 1: Initial Consultation and Bloodwork
Comprehensive Bloodwork: Just like the home program, you’ll start with a blood test. You can have this done at Purifi IV or a kit will be sent to you.
Initial Evaluation: After reviewing your bloodwork results, you’ll have a face-to-face consultation with our medical provider to determine if TRT is right for you.
Step 2: Qualification for TRT
If you’re a good candidate for TRT, our provider will discuss a personalized treatment plan with you.
This treatment plan will be recorded in our system, and you'll be scheduled for your first injection at Purifi IV.
Step 3: Monthly Subscription
Cost: $150/month
The monthly subscription includes bloodwork, testosterone injections administered at Purifi IV, and regular check-ins with our medical staff.
The subscription is month-to-month, and your payment will renew each month for continued care and treatment.
Step 4: Weekly Injections at Purifi IV
Clients who opt for in-clinic TRT will receive their testosterone injections weekly. Each session is supervised by our experienced registered nurses (RNs), and your progress will be monitored continuously to adjust your treatment plan as needed.
Important: If you start with home TRT and later decide to switch to in-clinic injections, you’ll need to finish your current prescription before transitioning. This ensures compliance with scheduled medication regulations.
Step 5: Ongoing Monitoring and Support
Stay on track with regular follow-ups, including bloodwork and any necessary dose adjustments. You’ll also have access to your provider for ongoing questions via phone and/or text and telehealth consultations.
Direct Communication with Your Medical Provider
Both TRT at Home and TRT at Purifi IV options include direct communication with your medical provider. Whether you prefer home-based care or in-clinic visits, you’ll always have access to your provider via phone and/or text for follow-up questions, treatment adjustments, and support.
