Maintenance of Chronic Medical Conditions
We Will Not See Or Evaluate Conditions Or Symptoms Considered Emergencies- We Will Instruct You To Go To The ER (Eg: Chest pain, open fractures, severe difficulty in breathing…etc)

Pricing

Monthly Plan:

$100/month with unlimited calls and texts to medical provider.

Home/Work Visit on monthly plan: $150/visit ($50-$150 in savings compared to As Needed Plans)

calling in/ e-prescribing on monthly plan- $10

As Needed Plans:

E-prescribing included with each of these plans

Bronze

-Site visit for flu, covid, strep swab - $200

Silver

-Site Visit for more complex medical ailments (GI symptoms, evaluation for UTI with urinalysis, more complex cold and flu symptoms, otoscope exams)- $250

Gold

-Site visit with procedure (eg: stitches, eye staining for foreign body evaluation) - $300

Platinum

Any of either the Bronze, Silver, or Gold Plans with addition of IV hydration- $400
