1. Acceptance of Terms

These Terms of Use ("Terms") govern your access to and use of the website located at purifiiv.com (the "Site"), operated by Purifi Clinics, LLC d/b/a Purifi IV ("Purifi IV," "we," "us," or "our"). By accessing or using the Site, booking an appointment, or otherwise interacting with our online services, you agree to be bound by these Terms. If you do not agree, please do not use the Site.

These Terms apply to your use of the website. Your in-clinic or mobile treatment is separately governed by the informed consent forms, financial policies, and Notice of Privacy Practices you sign prior to treatment.

2. About Our Services

Purifi IV is a physician-owned, physician-led IV hydration, vitamin infusion, injection therapy, aesthetics, and medically managed weight loss clinic located in Suwanee, Georgia, with mobile service available in the surrounding area. Medical Director: Dr. Jeremy Nelson, MD.

4. Appointments, Pricing & Promotions

Prices, service availability, and promotional offers (such as new-client discounts) displayed on the Site are subject to change without notice and may vary by location. Booking through the Site or a third-party platform (e.g., our online scheduler) does not finalize treatment — final eligibility is determined in-clinic by your provider. Our separate Refund & Cancellation Policy governs deposits, cancellations, and no-shows.

5. User Conduct

You agree not to:

  • Use the Site for any unlawful purpose or in violation of these Terms;

  • Submit false, inaccurate, or fraudulent information (including health information) through any form on the Site;

  • Attempt to gain unauthorized access to our systems, interfere with Site functionality, or introduce malicious code;

  • Scrape, copy, or republish Site content for commercial purposes without our written permission.

6. Intellectual Property

All content on the Site — including text, graphics, logos, the "Purifi IV" name and mark, drip/service names, and images — is owned by or licensed to Purifi Clinics, LLC and is protected by copyright and trademark law. You may view and print Site content for personal, non-commercial use only. No other use is permitted without our prior written consent.

7. Third-Party Links & Services

The Site may link to or integrate with third-party services, including our online scheduling platform, payment processors, and financing partners (e.g., Cherry). We do not control and are not responsible for the content, privacy practices, or terms of any third-party site or service. Your use of those services is governed by their own terms.

THE SITE AND ITS CONTENT ARE PROVIDED "AS IS" AND "AS AVAILABLE," WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, OR NON-INFRINGEMENT. WE DO NOT WARRANT THAT THE SITE WILL BE ERROR-FREE, UNINTERRUPTED, OR SECURE, OR THAT ANY INFORMATION ON THE SITE IS COMPLETE OR CURRENT.

9. Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW, PURIFI CLINICS, LLC, ITS OWNERS, PROVIDERS, AND STAFF SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING FROM YOUR USE OF THE SITE. THIS SECTION DOES NOT LIMIT LIABILITY FOR IN-CLINIC MEDICAL TREATMENT, WHICH IS SEPARATELY GOVERNED BY THE INFORMED CONSENT FORMS YOU SIGN PRIOR TO TREATMENT.

10. Indemnification

You agree to indemnify and hold harmless Purifi Clinics, LLC and its owners, employees, contractors, and agents from any claims, damages, or expenses (including reasonable attorneys' fees) arising from your misuse of the Site or violation of these Terms.

11. Governing Law & Disputes

These Terms are governed by the laws of the State of Georgia, without regard to conflict-of-law principles.

Binding Arbitration & Class Action Waiver. Any dispute, claim, or controversy arising out of or relating to these Terms or your use of the Site (excluding claims for injunctive relief related to intellectual property, and excluding any claims arising from in-clinic medical treatment, which are governed separately by your signed informed consent) shall be resolved by binding arbitration administered by the American Arbitration Association under its Commercial Arbitration Rules, rather than in court, except that either party may bring an individual claim in small claims court. The arbitration will take place in Gwinnett County, Georgia, unless you and Purifi IV agree otherwise. You and Purifi IV each waive the right to a jury trial and to participate in a class action, class arbitration, or representative action. If any part of this arbitration clause is found unenforceable, the remainder will still apply, and the dispute will proceed in the state or federal courts located in Gwinnett County, Georgia, where you consent to personal jurisdiction.

12. Changes to These Terms

We may update these Terms at any time. Changes are effective upon posting to the Site. Your continued use of the Site after changes are posted constitutes acceptance of the revised Terms.
